Core Idea

Electronystagmography (ENG) is a clinical eye-movement recording technique that infers gaze position from the standing electrical potential between the relatively positive cornea and negative retina. Electrodes placed around the eyes measure voltage changes as the eyes rotate. Calibrated traces quantify horizontal and, with greater limitations, vertical movement and nystagmus during gaze, oculomotor, positional, positioning, and caloric tasks.

The recognition invariant is corneo-retinal dipole + periocular electrodes + calibration + time-series eye-position signal + controlled vestibular/oculomotor task + bounded clinical interpretation. ENG is a transduction and test-battery method, not a disease diagnosis.

Scope of Application

ENG supports assessment of nystagmus, ocular motor performance, vestibulo-ocular reflex asymmetry, positional responses, and low-frequency horizontal semicircular-canal response during caloric testing. Professional audiology standards include selecting, administering, and interpreting ENG/VNG among vestibular competencies.

Its value is greatest as one component of a question-driven battery. Some patients need VNG, video head-impulse testing, rotary chair, vestibular-evoked myogenic potentials, imaging, or neurological evaluation to resolve findings that ENG cannot localize.

Clarity

The electrodes do not directly record vestibular nerve activity. They record a voltage field produced by eye rotation, so calibration and signal stability are load-bearing. A trace can be technically precise yet clinically misleading if the task, fixation state, medications, alertness, vision, or baseline ocular motor disorder is not controlled.

Manages Complexity

ENG compresses a fleeting eye-movement examination into synchronized, measurable traces. It permits comparison between directions, ears, stimulus conditions, and fixation states, and it preserves data for review.

The compression discards torsional detail and can confound nonocular electrical activity with eye motion. Interpretation therefore retains raw-trace inspection, calibration checks, artifact marking, and cross-test consistency rather than trusting one computed index.
