Medical Error¶
Classify a preventable failure of a planned clinical action—commission, omission, wrong plan, or correct plan executed incorrectly—whether or not harm results, and analyze it through the care process and defenses that allowed it or caught it.
Core Idea¶
A medical error is a preventable failure in planning or executing clinical care: the wrong plan is chosen, the right plan is carried out incorrectly, or a required action is omitted. Harm need not occur; near misses and intercepted failures preserve the error even when defenses prevent injury. It requires a clinical intention or standard, a deviation in planning or execution, preventability under the relevant conditions, and a care-process locus. Outcome severity, negligence, and culpability are separate judgments.
Scope of Application¶
Diagnostic, surgical, transfusion, communication, device, and monitoring cases share the structure because all are parts of clinical care. Removing clinical intention, patient exposure, and standard-of-care context leaves generic mistake or failure.
Clarity¶
An adverse event is an unfavorable clinical occurrence with causality still open. A preventable adverse event is harm caused by error. Complication can occur despite appropriate care. Medication error is one clinical-process subtype.
Manages Complexity¶
The abstraction turns a scattered family of cases into one role-based test: identify the required elements, test their relation, and reject the classification when a constitutive commitment is missing.
