Watchful Waiting¶
A clinical plan to defer a considered treatment while observing the condition and revisiting the decision if its course changes.
Core Idea¶
Watchful waiting is a chosen clinical strategy: a contemplated disease-directed treatment is deferred while the condition is observed, with a way to reconsider the choice if relevant symptoms or findings change. It is not simply absence of treatment or lost follow-up. Scope of Application¶
CDC describes a short watchful-waiting option for selected mild middle-ear infections before deciding whether antibiotics are needed. In a randomized trial of men with minimally symptomatic inguinal hernia, the waiting arm received symptom-reporting instructions and follow-up examinations while repair was deferred; increasing pain commonly prompted later repair. NCI's prostate-cancer usage can instead mean follow-up with palliative treatment for symptoms and no planned curative therapy. Clarity¶
State what treatment is deferred, how change will be noticed, and what finding reopens the decision. A fixed schedule of tests or one waiting duration is not universal. NCI cautions that oncology publications do not always consistently separate watchful waiting from active surveillance; compare the actual curative or palliative aim rather than assuming exact synonymy.[ref-46d65e1a97c2][ref-1ef05b297d44]
Manages Complexity¶
The strategy replaces a static treat/not-treat label with a conditional sequence: defer a considered intervention, observe relevant change, then reassess. That can avoid intervention that proves unnecessary, but delay can carry harm if important change is missed.
