A Competency Checkoff is a live, observed demonstration of a specific skill, judged by an authorized person who then signs off that the learner may proceed. Its defining feature is the human in the loop at the moment of performance: the evidence is a real enactment watched in real time, and the output is a named person's recorded attestation — a signature, a checked box with an initials line — that carries accountability. It is not a written probe and not a synthesized scenario; it is someone qualified saying "I watched you do this correctly, and I am releasing you." The signoff is the mechanism's product, which is why it is used where the stakes of a wrong release are borne by an accountable observer.
Example¶
In a nursing school's skills lab, students cannot practice on patients until they are checked off on peripheral IV insertion. The checkoff is concrete: the student performs the full procedure on a training arm while a clinical instructor watches — hand hygiene, vein selection, angle of insertion, flash confirmation, securing the line, sharps disposal — each step observed against a fixed list of must-pass actions. The instructor does not interrupt to teach; she watches the live sequence and judges whether it meets the standard.
If every critical step is performed correctly, she signs and dates the student's checkoff card, and that signature is what unlocks supervised patient practice. If the student contaminates the field, she withholds the signoff and the student returns to practice before re-attempting with an instructor. The record is auditable: months later, anyone can see who attested that this student could safely place an IV, and when. That accountable signature — not a percentage — is what the next stage relies on.
How it works¶
The mechanism's engineering is about observing the real act and attaching accountability to the release. The skill is decomposed into observable, must-pass actions, and an authorized observer watches a single continuous live performance against them — presence and correctness of each critical step, not a score curve. Judgment is typically all-or-nothing on critical items: a missed sterile step fails the whole demonstration regardless of the rest. When the performance meets the standard, the observer records a signed, dated attestation naming who released whom for what. That signed release is the gate; downstream stages trust it because a qualified person's name is on it. The checkoff deliberately does not construct an artificial high-stakes scenario or validate that scenario's realism — it observes the genuine task as normally performed.
Tuning parameters¶
- Observer authority — who is qualified to sign. Restricting signoff to experts raises trust but creates bottlenecks; widening it speeds throughput but risks lenient releases.
- Critical-item strictness — which steps are must-pass versus advisory. More all-or-nothing items protect safety but raise failure rates on minor slips.
- Attempts before escalation — how many live tries before a learner is routed to remediation or review. Generous attempts reduce anxiety; too many normalize repeated failure.
- Attestation weight — how much accountability the signature carries (informal check vs. formally auditable record). Heavier attestation deters rubber-stamping but slows the observer down.
When it helps, and when it misleads¶
Its strength is that it certifies doing, watched live, with a named person accountable for the release — the level of Miller's pyramid where a learner "shows how" under direct observation rather than merely describing.[1] For hands-on, safety-relevant skills, nothing a written test yields substitutes for an expert watching the actual act.
It misleads through the observer. The classic misuse is the rubber-stamp signoff: a rushed or conflict-averse observer signs to keep the line moving, and the attestation becomes a formality rather than a judgment. Inter-observer drift is the quieter failure — two instructors' bars diverge, so the same performance passes with one and fails with another. The guarding discipline is observer calibration and periodic co-observation, plus keeping the critical-item list explicit so a signature means the same thing regardless of who signs.

assessment_evidence— the live, observed enactment of the skill against must-pass actions is the evidence; it is watched, not inferred from an artifact.progression_gate— a completed signoff is what unlocks the next stage (supervised practice, here); no signoff, no entry.advancement_release_decision— its signature product: an accountable, recorded attestation of who released whom, for what capability, and when.
It does not implement gate_validity_review or the staging of prerequisite_capability under synthesized conditions — that is Simulation Checkout, its nearest twin, which builds a controlled scenario and checks that the scenario predicts real readiness, whereas a checkoff observes the genuine task performed live.
