Late-Entry and Backfill Protocol¶
Documentation protocol — instantiates Capture-Latency Evidence Stratification
A procedure for adding a late or corrected entry so it is marked as backfill with its author, time, and basis, never fused into the primary record.
A Late-Entry and Backfill Protocol governs the single riskiest act in a stratified record: adding something after the fact. Its defining stance is permissive but honest — late correction is allowed, even encouraged, but it must arrive wearing a badge that says late. The protocol fixes, at the moment of entry, the rules for how a delayed addition is recorded: it goes in as a new, dated, attributed entry, never by editing the original; it states when it was written, by whom, and on what basis; and it is visibly distinguished from what was captured live. The mechanism is the discipline that prevents a backfilled detail from silently acquiring the authority of a contemporaneous one. Its concern is not the content of the late note but its legibility as late.
Example¶
A nurse realizes at 4 p.m. that she forgot to chart a patient's 11 a.m. medication. The wrong fix — and the one the protocol exists to forbid — is to open the 11 a.m. slot and type the dose in as if it had been recorded then. The right one: she creates a late entry, timestamped 4 p.m., reading "Late entry for 11:00 — 5 mg administered per MAR and my direct recollection; charting delayed due to code in adjacent room." The record now shows two times — the event time (11 a.m.) and the entry time (4 p.m.) — plus who wrote it and why.
Six months later, if the chart is reviewed, the difference is unmistakable: the dose was real, but it was documented five hours late, from memory, under load. That is materially different from a dose charted live, and the protocol guarantees the difference survives. It never erases the gap; it labels it.
How it works¶
- Never overwrite. A correction or addition is a new entry that references the original, which stays intact. The primary layer is left for other mechanisms to preserve; this protocol simply refuses to touch it.
- Dual timestamps. Both event time and entry time are recorded, making the latency of the addition explicit on the entry itself.
- Attribution and basis. Every late entry names its author and states its evidentiary basis and reason — direct memory, a found document, a corrected calculation — so a reader can weigh it.
- Visible marking. The entry is flagged as backfill in whatever channel a reader will encounter it, so it can never be mistaken for a live capture.
Tuning parameters¶
- Basis strictness — from "reason optional" to "documented evidentiary basis required." Stricter capture makes late entries more trustworthy but adds friction that can suppress honest correction.
- Latency disclosure granularity — exact dual timestamps versus a coarse "entered late" flag. Exact times carry more information; a flag is lighter but hides how late.
- Correction vs. addendum distinction — whether the protocol separates "fixing an error" from "adding new information," which downstream readers may treat very differently.
- Approval requirement — whether high-stakes backfills need a second signer. Sign-off raises integrity and slows the record.
When it helps, and when it misleads¶
Its strength is that it resolves the archetype's core tension directly: records genuinely need late correction, and this protocol lets them have it without letting correction masquerade as contemporaneity. A protocol that is easy to bypass is decorative. The classic misuse is the "cleaned-up" record produced under scrutiny, where late reconstructions are folded into original slots to present a tidier history. The guarding discipline is to make the compliant path the easy one and the bypass path detectable — pairing this protocol with an immutable original store so that overwriting is not merely against policy but technically evident.
