A foresight method that imagines the effort has already failed and works backward to surface the divergent risk models people were holding silently.

Before a hard undertaking begins, people often hold private, incompatible pictures of how it might go wrong — and social pressure to sound committed keeps those worries unspoken until they come true. A Pre-Mortem is the method that pulls them out early by inverting the frame: it asks the team to assume the effort has already failed, and to work backward to explain why. Its defining move is that failure is stipulated, not debated — which flips the task from optimistic advocacy to the safe generation of reasons things went wrong, and makes each person's private risk model expressible. When those imagined-failure accounts are compared, the places where they diverge are exactly where people were holding different mental models of the risks. It aligns not by asserting a shared picture but by mining imagined futures for the disagreements that were already there.

Example

Before breaking ground on a transit-tunnel megaproject, the delivery team runs a pre-mortem, following the structure Gary Klein described.[n1] The facilitator sets the frame: "It's three years from now and this project has failed — over budget, behind schedule, publicly. On your own, write down every reason why." Working independently first, engineers cite a geotechnical surprise the boring machine can't handle; the finance lead writes down a currency swing on imported equipment; the community-relations lead writes down a lawsuit stalling the portal site; a junior planner quietly notes that two agencies each assume the other owns utility relocation. Read aloud and clustered, the accounts reveal that these are not one shared risk model but four — and the utility-relocation gap is a live misalignment nobody had voiced.

The team converts the top divergences into agreed watch-signals and owners: a monitored leading indicator for the geotechnical risk, an explicit assignment for utility relocation. The value came from surfacing, before the plan locked, the failure stories people were each privately carrying.

How it works

  • Stipulate the failure. Frame it as a fact — "the project has failed" — so people generate reasons rather than defend the plan.
  • Generate independently first. Have each person write reasons alone before any discussion, so the loudest or most senior read does not anchor everyone else.
  • Cluster and compare. Pool the accounts and group them; the clusters where people diverge mark the differing risk models the method is built to find.
  • Convert to signals and owners. Turn the top divergences into leading indicators to monitor and mitigations with named owners, so the surfacing changes the plan.

Tuning parameters

  • Independence of generation — silent solo writing versus open group brainstorm. Solo generation defeats groupthink and surfaces private dissent; group brainstorm is livelier but lets strong voices set the frame.
  • Failure framing — total failure versus a specific partial failure. "Everything failed" casts the widest net; a targeted "the launch failed" focuses the search but can miss orthogonal risks.
  • Participant breadth — core team only versus including junior voices, skeptics, and adjacent functions. Breadth catches the assumption the core team shares and cannot see; narrowness is faster but blinder.
  • Conversion rigor — whether surfaced risks become owned watch-signals or just a list. High rigor changes the plan; a logged-and-forgotten list is theater.

When it helps, and when it misleads

Its strength is that it legitimizes dissent by making pessimism the assignment — prospective hindsight surfaces concerns people would never volunteer in a status meeting, and comparing the divergent failure stories exposes risk-model mismatches while the plan can still change.

Its central failure mode is that it degenerates into a gripe session or a recital of generic, un-owned risks that changes nothing — surfaced and logged, never converted to action. The classic misuse is running it after the plan is locked, so it functions as ritual reassurance rather than a genuine check. The discipline that guards against this is independent generation before discussion, and a hard requirement that the top divergences leave the room as owned watch-signals and mitigations, not as a list.

How it implements the components

Pre-Mortem fills the surface-divergence components — the ones a prospective failure-imagination method produces:

  • assumption_comparison — independently-generated failure accounts expose where actors' risk models differ; the method's core work is comparing them.
  • misalignment_signal — the divergent failure modes become the leading indicators the team agrees to watch for, converting silent disagreement into monitored signals.

It does not test the model by enacting it (coordination_rehearsal) — that is Coordination Rehearsal Session, which physically runs the plan where the pre-mortem only imagines it; and it does not map roles and constraints or maintain a standing reference (role_constraint_state_map, shared_reference_model) — those belong to Cross-Functional Planning Session and Common Operating Picture. Unlike the retrospective After-Action Review, which learns from a failure that actually happened, the Pre-Mortem works from a failure that is only imagined, before the effort begins.
