Retrospective Synthesis¶
Reflection workflow — instantiates Accumulation Compaction
Distills many incidents or episodes into a small set of recurring patterns and forward commitments — deliberately letting individual detail recede within a loss budget, while reviewing whose cases get represented so the lessons are not skewed.
Retrospective Synthesis compacts a large pile of episodes — incidents, projects, complaints, experiments — into a small set of patterns and commitments meant to guide future action. Its defining orientation is forward-looking and lossy: it does not aim to keep each episode reachable, but to answer "what keeps happening, and what should we do about it?" The output is a handful of recurring themes and the actions they warrant, and getting there means deliberately letting most of the individual detail recede — an explicit loss budget on how much of each case survives into the lesson. Precisely because so much is dropped, the choice of which episodes become lessons is where distortion enters, so a bias review of whose cases are represented is not decoration but the load-bearing safeguard against a synthesis that flatters the loudest voices.
Example¶
A hospital's patient-safety office has 300 incident reports from the past year — medication errors, near-misses, equipment faults. Read individually they are a blur; no clinician can act on 300 stories. Twice a year the office runs a synthesis. Reading across the reports, it distills five recurring patterns: most medication errors cluster around shift-change handoffs; a specific infusion pump features in a disproportionate share of near-misses; and so on. Each pattern comes with a forward commitment — a handoff checklist, a pump replacement — and the year's 300 reports become a five-point action agenda the safety committee can actually pursue.
Two safeguards keep it honest. A stated loss budget acknowledges up front that individual case nuance is being sacrificed for actionable patterns — the synthesis is not the record, and it says so. And a bias review checks that the patterns are not driven only by the units that report most diligently: a quiet ward with real problems but few filed reports must not vanish from the picture just because it generated less paperwork. The synthesis guides next year's priorities without pretending to preserve every incident.
How it works¶
What distinguishes it from ordinary summarizing is that it is lossy distillation with a bias check on representation:
- Read across, not down. Look for recurrence and cause across the whole set of episodes rather than describing each one — the goal is patterns, not a catalog.
- Distill into patterns and commitments. Reduce the pile to a few themes, each paired with the forward action it warrants.
- State the loss budget. Declare that individual detail is being sacrificed and that the synthesis is a guide, not the source record.
- Review for representational bias. Check whose episodes drove the patterns, and correct for over- and under-reported sources before the lessons are trusted.
Tuning parameters¶
- Pattern count — how many themes the synthesis distills to. Fewer themes are more actionable but discard more; more themes are faithful but dilute focus.
- Loss budget — how much individual case nuance may be sacrificed. A tight budget keeps synthesis close to the evidence but resists real compression; a loose one yields crisp lessons that can outrun what the cases support.
- Bias-review rigor — from an informal gut check to a structured audit of source representation. More rigor guards against skewed lessons but costs effort; less risks canonizing the squeaky wheel.
- Commitment firmness — whether patterns produce binding actions or advisory observations. Firm commitments drive change but can over-fit to a noisy year; soft ones inform without forcing.
When it helps, and when it misleads¶
Its strength is that it converts an unusable mass of episodes into a short, forward agenda a group can act on — the move that turns raw incident history into prevention priorities. The disciplined form of it is the after-action review, whose whole point is extracting transferable lessons rather than archiving what happened.[n1]
It misleads when the distillation quietly encodes bias. Synthesis chooses which episodes become the story, so patterns can reflect who reports most, who is most vivid, or what the synthesizer expected to find — and a confident five-point agenda can enshrine that skew as fact. This is the archetype's biased-summary failure mode at its sharpest, which is why the bias review is central rather than optional here. It also fails when readers treat the lessons as the complete record and act as if the underlying episodes no longer matter — the "false finality" trap. The discipline is to name the loss budget openly so the synthesis is not mistaken for the archive, and to run the bias review on representation before any commitment is set. Keeping every source episode individually reachable and provenance-certified is a different job — that navigable, evidence-linked layer over preserved sources is Archival Summarization.

Retrospective Synthesis fills the lossy-distillation side of the archetype — turning many episodes into a few actionable lessons:
