When a supervisor pushes to send a crew in to inspect the line "while it's fresh," the checklist answers with an unchecked box — atmospheric readings are still above the limit — and the gate stays shut. How it works

  • Enumerate the floor as binary items. State each minimum condition — isolation, containment, life safety, continuity of essential utilities — as a checkable, unambiguous item, not a paragraph of guidance.
  • Require all-green to pass. Treat the checklist as conjunctive: every item must hold. A single unchecked box holds the gate shut regardless of pressure.
  • Gate the phase transition. Completion is the reentry rule out of stabilization — the explicit, auditable criterion that licenses the move to restoration.
  • Re-run on regression. If a floor condition later fails, the gate reverts and the system drops back to stabilization rather than pressing on.

Tuning parameters

  • Floor height — how much must be true before proceeding. A higher floor is safer but delays restoration; too high and the system never gets to rebuild.
  • Item strictness — hard pass/fail versus judgment-based items. Strict binaries resist pressure and are auditable; judgment items fit messy reality but can be argued open.
  • Scope — a lean life-safety core versus a broad floor including continuity and records. Broader floors prevent more secondary harm but slow the gate.
  • Override policy — whether, and by whose authority, an unchecked item can be waived. Any override reintroduces exactly the premature-reentry risk the checklist exists to prevent.

When it helps, and when it misleads
