The Diagnostic Story

Symptom: A needed intervention is visible to multiple people but nobody acts, because each assumes someone else is handling it or is unsure whether they are the right person. Alternatively, the most confident or most senior person intervenes outside their competence while a qualified actor stays passive because the situation is socially ambiguous. After-action reviews consistently find that responsibility was nominally shared but not operationally assigned.

Pivot: Define an explicit action condition that links situational appraisal, competence verification, scope authority, and escalation path — then embed that condition in training, workflow, cues, and review so competent actors activate promptly and non-competent observers still route the problem to competence.

Resolution: Bystander delay decreases and unsafe interventions by actors outside their scope decrease. Handoffs from observers to qualified responders become clearer and more reliable. After-action learning improves because activation and non-activation are observable and reviewable.

A needed intervention is visible to multiple actors, but no one can confidently determine whether they are the right person to act. Some observers under-act because responsibility is diffuse; others over-act because confidence, status, or urgency is mistaken for competence. The system lacks a legitimate trigger that connects observed need to verified capability.

What this problem means

Bystander ambiguity and competence uncertainty reinforce each other. The more people who can see a problem, the easier it is for each person to assume another person is responsible. At the same time, the more specialized the action, the more reasonable hesitation becomes. Without an explicit competence condition, hesitation can be mistaken for prudence and overreach can be mistaken for initiative.

The structural problem is therefore not only “people fail to act.” It is that the system lacks a legitimate mapping from observed condition to qualified actor to safe action or handoff.
