The Diagnostic Story

Symptom: Growth metrics look strong while quality, reliability, and support capacity quietly deteriorate behind them. New sites, cohorts, or regions launch faster than training, governance, and oversight can absorb, so the original conditions that made the pilot successful no longer hold at the larger scale. Leadership reads the absence of visible catastrophe as a green light for wider expansion, even as incident rates, rework, support burden, and staff overload accumulate as hidden leading indicators.

What this problem means
