The Diagnostic Story

Symptom: Important risks arrive at the overseer late, after harm or reputational damage has already occurred. Supervisors cancel reviews, coaching, and field visits because exception handling consumes all available attention; approval queues pile up at the same bottleneck; high performers and struggling workers are treated as equivalent load because there is no time for differentiated response.
