Reflexive Self Monitoring¶
Enable a system or actor to observe its own behavior and use that observation to adjust future behavior.
The Diagnostic Story¶
Symptom: The same issue appears in every retrospective, every incident review, every coaching cycle — and still nothing changes in the underlying behavior. Actors are surprised by their own patterns when evidence is finally shown to them. Reviews produce insight but no concrete experiment or accountability. Metrics exist, but the people they describe treat the data as external judgment rather than as information about themselves.
Pivot: Create a bounded self-observation and adjustment loop: define what the actor needs to notice about itself, represent that evidence in a form the actor can own, review it at a usable cadence, decide what to change, and preserve memory of whether the change worked.
Resolution: Repeated harmful patterns become visible before they harden into culture or normal operation. Self-correction becomes more timely and less dependent on crisis or external blame. Experience converts into changed practice rather than repeated anecdote, and accountability becomes more evidence-informed because the actor is working from its own record.
Reach for this when you hear…¶
[agile coaching] “We've retrospected on 'communication breakdown' eleven sprints in a row and never changed anything structural — at some point we have to stop naming the symptom and look at what we're actually doing differently now.”
[clinical supervision] “The trainee was shocked when I played back the recording of them interrupting the patient twelve times in eight minutes — they genuinely believed they were listening well.”
[executive coaching] “The 360 feedback was the first time he'd ever seen his own behavior from outside his own head, and it was genuinely useful precisely because he didn't get to curate it.”
When This Archetype Applies¶
Partial catalog groundingSome structural conditions are represented by existing abstractions, but no sufficient condition set is fully represented.
Diagnostic problem
The system or actor affects itself over time but lacks a usable representation of its own behavior, state, pattern, or decision process, so harmful routines, blind spots, drift, and repeated mistakes persist.
What this problem means
The structural problem is self-blind repetition. A system affects itself over time, but it lacks a usable representation of its own behavior, state, decision process, or contribution to outcomes. Because the pattern remains invisible or poorly represented, the actor treats each episode as isolated, blames the wrong cause, or repeats the same ineffective correction.
The failure is often not ignorance of final outcomes. A team may know deadlines are missed, a student may know answers are wrong, a service may know incidents recur, and an organization may know people are dissatisfied. What is missing is a usable view of the process that creates those outcomes.

Opaque generative habits · open
People can describe outcomes but cannot describe the habits, process choices, triggers, or decisions that keep producing those outcomes.
What is missing is a usable view of the process that creates those outcomes. The narrower requirement in this condition set is: People can describe outcomes but cannot describe the habits, process choices, triggers, or decisions that keep producing those outcomes.
Self-aware adaptation need · grounded
Learning, adaptation, or behavior change depends on recognizing one's own uncertainty, strategy, progress, or deviation.
Use this archetype when the same person, team, organization, or system both contributes to the pattern and has some ability to change its future behavior. The narrower requirement in this condition set is: Learning, adaptation, or behavior change depends on recognizing one's own uncertainty, strategy, progress, or deviation.
Externally owned monitoring · open
External monitoring exists but the monitored actor does not own or use the evidence.
This is a load-bearing situation condition in the diagnostic expression. The condition is: External monitoring exists but the monitored actor does not own or use the evidence. If it does not hold, this particular condition set is incomplete.
Other requirements and context (2)
Why these sit outside the expression
Supporting context — it may accompany or help interpret the situation, but it is not a load-bearing condition in a sufficient diagnostic set.
Supporting contextThe actor repeatedly encounters the same failure, conflict, lapse, delay, confusion, or drift.
The system or actor affects itself over time but lacks a usable representation of its own behavior, state, pattern, or decision process, so harmful routines, blind spots, drift, and repeated mistakes persist. In this archetype, the relevant contextual consideration is: The actor repeatedly encounters the same failure, conflict, lapse, delay, confusion, or drift. It helps interpret the situation or strengthens the practical case for examining the archetype.
Supporting contextA team or organization wants to improve its operating process but relies on anecdote, blame, heroic effort, or after-the-fact rationalization.

Mechanisms / Implementations¶
- Habit Tracker: Records recurring personal behavior so patterns, streaks, lapses, triggers, and adjustment opportunities become visible.
- Personal Dashboard: Aggregates selected self-observation signals for an individual, operator, learner, or role holder.
- Team Retrospective: Creates a recurring space for a team to inspect its own work process, coordination patterns, and improvement commitments.
- Postmortem Review: Uses a failure, incident, near miss, or major outcome as evidence for revising future behavior and safeguards.
- Self-Assessment Tool: Helps an actor compare its own performance, process, or state against criteria before external judgment or final outcome.
- Metacognitive Prompt: Asks the actor to notice its own thinking, uncertainty, attention, assumptions, or strategy while acting or learning.
- Organizational Review: Periodically examines an organization's own routines, metrics, norms, commitments, and outcomes to revise how it operates.
- Reflective Journal: Preserves episodes, interpretations, emotions, strategies, and follow-up commitments for later pattern recognition.
- Peer Feedback Session: Adds other observers to help the actor see behavior that self-observation alone may miss.

Variants¶
Narrower or domain-specific specializations that share this archetype's core structure. Recognized variants are established; candidate variants are provisional.
Personal Self-Tracking Loop · scale variant · recognized
An individual records selected behavior, state, or context and uses the pattern to adjust habits, routines, attention, or choices.
Team Retrospective Loop · governance variant · recognized
A team periodically inspects its own coordination, decision process, workload, and working agreements, then commits to changes.
Postmortem Learning Loop · risk or failure variant · recognized
After a salient outcome, incident, near miss, or failure, the system examines its own contribution and changes future safeguards or behavior.
Organizational Reflexive Review · governance variant · recognized
An organization examines its own norms, routines, metrics, decision structures, and outcomes, then changes how it governs itself.
Editorial Notes¶
Problem Classification¶
Classification: Observability, Measurement & Feedback Gaps → Hidden State, Structure & Trajectory Visibility
Problem kernel: a self-affecting system cannot see its own recurring pattern
Rationale: Earliest causal condition: The system or actor affects itself over time but lacks a usable representation of its own behavior, state, pattern, or decision process, so harmful routines, blind spots, drift, and repeated mistakes persist.
Independent corroboration: The earliest necessary condition in the frozen evidence is: The system or actor affects itself over time but lacks a usable representation of its own behavior, state, pattern, or decision process, so harmful routines, blind spots, drift, and repeated mistakes persist. That is a hidden state structure and trajectory visibility problem because Internal state, dependencies, informal structure, emerging patterns, or available action paths remain invisible to actors who must diagnose and act.
Review outcome: Independent reviewer agreement; high confidence.
