The Diagnostic Story

Symptom: The standard response is being applied to cases that differ from each other in consequential ways, and exceptions pile up or require constant manual escalation. A small group must handle all variation because no differentiated repertoire exists. The system performs acceptably in normal conditions and collapses when variation appears.

Pivot: Map the variety the environment actually presents, then map the response repertoire that currently exists. Add, differentiate, or route responses to cover the relevant variety — matching the range of incoming disturbances with a matched range of prepared answers.

Resolution: Unhandled exceptions decrease because responses now exist for recurring disturbance classes. Control overload eases as cases are routed to appropriate responses rather than all escalating to the center. What this problem means

The structural problem is a mismatch between environmental variety and internal response variety. The environment presents varied cases, disturbances, risks, needs, or states; the system has too few distinctions, too few response modes, poor routing, weak escalation, or stale procedures. The result is brittle control. Ordinary cases may be handled well, but unusual or complex cases trigger delay, manual workarounds, unsafe simplification, unnecessary escalation, or collapse.

A second structural problem is the opposite failure: uncontrolled response variety. Systems sometimes create many special procedures without governance, ownership, training, or retirement. That kind of variety is not requisite; it is noise. The archetype preserves only variety that improves fit, control, safety, learning, or resilience.
