Closability Scoring Rubric answers the question the raw gap cannot: of the shortfall we have measured and split, how much of it can we actually close? "Reduce readmissions" treats all of these as equally attackable; the rubric refuses to. Medication reconciliation scores high closability: strong evidence, fully within the team's control, low cost — a clear latent-capacity signal. Social circumstances score low closability from here: real, but largely outside the discharge team's control. The clinically unavoidable chunk scores irreducible, and is explicitly set aside so no one wastes effort on it. How it works

  • Define the closability dimensions. Fix the criteria every portion is scored on — evidence of movability, degree of control, cost to act, risk — before scoring, so the rubric is stable.
  • Score each decomposed portion. Apply the same levels to each gap channel, yielding a closability class rather than a yes/no.
  • Surface the latent-capacity portions. Portions that score as movable, controllable, and worth it are flagged as recoverable latent capacity — the ones genuinely worth someone's attention.
  • Set aside the irreducible and the artifactual. Hard limits and measurement artifacts are labeled so they stop absorbing improvement energy.

Tuning parameters

  • Rubric dimensions — which axes count (evidence, control, cost, risk) and their weights. More axes capture nuance but slow scoring and invite disagreement.
  • Band granularity — a coarse closable/not-closable split or a fine multi-level scale. Finer scales discriminate better but demand more evidence per portion.
  • Evidence bar — how much proof a portion needs to score as closable. A high bar prevents wishful "we could fix that" scores; too high freezes everything as uncertain.
  • Calibration protocol — whether multiple assessors score independently and reconcile. Reconciliation curbs single-assessor bias at the cost of time.

When it helps, and when it misleads

Its strength is that it stops improvement programs from chasing every shortfall equally: by scoring closability explicitly, it concentrates effort on recoverable latent capacity and openly retires the irreducible. The shared rubric also makes the judgment contestable — you can argue with a score's evidence rather than with someone's gut.

Its failure mode is a rubric that launders intuition: if the criteria are vague or the evidence bar is low, "closability" scores just re-encode the assessor's prior beliefs in a table that now looks objective. A classic misuse is scoring a politically-favored portion as highly closable on thin evidence to justify a pre-chosen initiative. The guarding discipline is a real evidence bar and independent calibration — a portion earns a "closable" score only when evidence, not enthusiasm, supports it.
