Binds every published finding to the vantage that produced it and the scope it is licensed to cover, so no claim travels downstream stripped of the caveat that it is only the landscape as seen from here.

A finding is born scoped — "readmissions, among patients who come back to this system" — and dies unscoped, quietly promoted to "readmissions" as it is copied into a slide, a target, a press line. Coverage-Limited Claim Register is the standing ledger that fights that decay: it records each claim as a row bound to the vantage that generated it, the scope of landscape that vantage can actually license a statement about, and the person answerable for that binding. The register does not fix that gap — but it stops the 11% from silently becoming a ranking.

How it works

  • Register at the claim, not the report. Each finding gets a durable entry keyed to its producing vantage, so the caveat is stored with the number rather than in the prose around it.
  • License a scope, not a disclaimer. The entry states the slice of landscape the vantage can support a claim about — the population, region, or condition inside its reach — which is narrower than the claim's surface wording.
  • Attach an owner. A named person is accountable for keeping the scope current and for adjudicating reuse requests.
  • Enforce inheritance on reuse. A rule travels with the claim: any derived artifact carries the same scope forward, and out-of-scope uses are blocked or flagged rather than left to the reader's memory.

Tuning parameters

  • Registration granularity — every metric versus only load-bearing ones. Finer coverage catches more scope creep but turns into bureaucracy nobody reads; register the claims that get reused as if unscoped.
  • Inheritance strictness — advisory footnote versus hard block on out-of-scope reuse. Strict inheritance prevents overreach but slows legitimate work and invites workarounds.
  • Scope vocabulary — free text versus a controlled set of population/region/condition tags. Structured scopes are machine-checkable and enforceable; free text is faster to author but decays into ignorable prose.
  • Refresh and expiry — how often an entry's scope is re-confirmed and whether stale claims auto-flag. Without expiry the register slowly fills with scopes that no longer match the vantage.

When it helps, and when it misleads

Its strength is that it attacks the exact failure the archetype names — a scoped output being read as the whole landscape — at the moment of reuse, which is where the promotion actually happens and where a caveat buried in the original report has long since fallen off. It gives the caveat a home that outlives the document and a person who owns it.

Its failure mode is that a register only preserves the limits someone already knew to record; a blind zone nobody has mapped never becomes a row, so the register can lend a false sense of completeness — "if it were limited, it'd be in the register." It also decays: unmaintained, its scopes drift out of date and it becomes ritual paperwork. And it is easily run for appearance — populated after the fact so a body of work looks rigorously scoped without any real constraint on reuse. The discipline is to make scopes machine-checkable so inheritance is enforced rather than trusted, to expire stale entries, and to treat the register as a floor on known limits, not a certificate that a claim's external validity has been established.[n1]
