Handoff Target Trace¶

Shared attention that lives only in the heads of the people present dies the moment they leave. Handoff Target Trace is the artifact that survives the departure: a written record of what the target is, the evidence behind it, its current state, and the focus the outgoing group had agreed on — captured so an incoming person can re-enter the same attentional frame rather than reconstruct it from scratch. Its defining move is persistence across a personnel boundary. A live callout aligns whoever is in the room now; a trace aligns whoever arrives next, which is why it is a document and not a spoken ritual — it is built to be read cold, by someone who was not there.
Example¶
An ICU nurse is ending a twelve-hour shift and handing off a deteriorating patient. Instead of a hurried verbal summary, she completes the handoff trace: the target ("watching for sepsis on Bed 4, Mr. A."), the evidence ("lactate up from 1.8 to 3.1 over six hours, MAP trending down, two units ordered"), the current state ("second fluid bolus running, cultures pending"), and the agreed focus ("if MAP drops below 65 again, escalate to the intensivist — do not wait for the culture result"). The incoming nurse reads it, and within a minute is attending to the same signal the outgoing nurse was watching, with the same trigger line, rather than starting her own read of the chart from zero. The trace is what carries the attentional frame across the shift boundary intact.
How it works¶
- Fix the target in writing. The record states, unambiguously and in durable form, what is to be attended to — not "keep an eye on Bed 4" but the specific signal and threshold.
- Attach the evidence and state. It carries the trail behind the target (trends, readings, prior decisions) and where things stand right now, so the reader inherits context, not just a label.
- Encode the role-specific focus. It names what the incoming role should notice and act on — the trigger conditions and the agreed next move — tailored to who is picking it up.
- Read to re-enter. The joiner reads cold and confirms they hold the frame; the trace, not a memory, is the source of truth.
The document's discipline is legibility to an absent reader: everything a present colleague would have said out loud must be on the page.
Tuning parameters¶
- Trace depth — a terse target line versus target-plus-evidence-plus-state. Deeper traces transfer richer frames but take longer to write and read; match to consequence.
- Structure rigidity — a free-text note versus a fixed template. Templates guarantee the critical fields are present but can flatten nuance.
- Focus specificity — how sharply the agreed next action and trigger lines are stated. Specific triggers travel cleanly; vague ones invite the joiner to re-interpret.
- Freshness window — how recently the trace must be updated to be trusted. A stale trace is worse than none because it looks authoritative.
When it helps, and when it misleads¶
Its strength is that it defends the most error-prone seam in any continuous operation — the handoff — where attention is silently dropped and rebuilt. Structured handoff tools such as SBAR exist precisely because unstructured verbal handoffs lose the target and its context.[n1] The trace turns "you had to be here" into a frame anyone qualified can step into.
It misleads when it is written to discharge the obligation rather than to transfer the frame — a box-ticked trace that is technically complete but omits the one judgment that mattered, giving the joiner false confidence they have the full picture. Its classic misuse is trusting a stale trace whose current-state section no longer matches reality. The guarding discipline is to write for the absent reader's decisions, not the writer's convenience, and to timestamp and refresh so the state section can be trusted.

It does not actively test, in the moment, whether the joiner is now attending correctly — the attention_drift_detector and joint_attention_readiness_scan components belong to Joint Gaze or Attention Probe; the trace supplies the frame to read, not the live check that it landed.
