Practice is supposed to move a learner toward mastery, but the wrong practice moves them the other way — each repetition grooving a misconception deeper instead of correcting it. Example

A student learning to add fractions keeps writing ½ + ⅓ = ⅖. The first time, the teacher lets it pass — anyone can slip. But the next several problems show the same shape: ¼ + ⅕ = 2/9, ⅔ + ⅙ = 3/9. Now it clears the loop's trend window — three consecutive problems, same systematic move — so it counts as divergence, not a slip. The diagnosis frame does not just mark the answers wrong; it reads the rule the student is actually running: they are adding numerators and adding denominators, a coherent but incorrect procedure. That specific misconception selects the correction. A generic "try again" would only groove the bad rule further, so instead the remediation re-grounds the concept — fraction bars showing why the denominator can't simply add — then re-sequences practice to common-denominator cases first, with feedback that targets the exact wrong step. The recovery target is set deliberately low: not "mastered all fraction addition," but "adds fractions with unlike denominators correctly on three fresh problems," at which point the student returns to the normal lesson flow. What the loop prevented was a month of homework silently reinforcing a wrong algorithm until it felt right.

How it works

  • Watch for a systematic pattern, not a single miss. Aggregate errors across several attempts; the trend window is what separates a persistent wrong procedure from an isolated mistake.
  • Diagnose the misconception, not just the wrongness. Infer the incorrect rule the learner is applying — the frame reads why the answers are wrong, because the same wrong answer can come from different bad rules.
  • Change the instruction to fit the cause. Re-explain, re-sequence, add scaffolding, or sharpen feedback — a correction aimed at the diagnosed misconception, never a bare "repeat."
  • Set a recovery target and re-enter. Define the modest "back on track" evidence that lets the learner rejoin normal instruction, then hand control back.

Tuning parameters

  • Trend window length — how many errors establish a real pattern before remediating. Short windows catch misconceptions early but over-remediate on flukes; long windows let bad rules entrench.
  • Diagnostic depth — a quick error-type label vs. a full interview into the learner's reasoning. Deeper diagnosis targets the correction precisely but costs instructional time.
  • Correction dosage — a nudge and one worked example vs. a full concept re-teach and re-sequence. Heavier dosage fixes deep misconceptions but interrupts momentum and can feel remedial.
  • Recovery-target height — how much re-demonstrated mastery ends the loop. A low bar returns learners fast but risks premature exit; a high bar is thorough but stalls progress.

When it helps, and when it misleads

Its strength is catching negative learning — practice that entrenches error — while the misconception is still soft, and correcting the cause rather than the symptom.
