Example

A coastal Vessel Traffic Service watches ship movements in a busy strait. One night two vessels come far closer than they should, and a controller catches it just in time to issue a warning. No collision, no damage — a clean incident report would file it as a non-event. The Near-Miss Notice Review does the opposite: it treats the near as the most informative thing that happened all week and reconstructs how the closing was almost missed.

The radar contact was there, but it blended into shore clutter on that bearing; the closing-speed alarm was set too loose to fire; the controller happened to glance over during a lull. Each is a fixable thinning of the watch. The review's product is a set of changes routed back into the system — a clutter-filter tweak so that bearing reads cleaner, a tightened closing-speed threshold, a note added to the scan sweep for that known pinch-point — so the next near-collision trips something instead of relying on a lucky glance. It also flags that the same alarm, once tightened, will cry wolf more often, and books that trade-off for the next tuning.

How it works

  • Harvest the near-misses. Actively surface the signals that were almost missed — late catches, lucky catches, one-person catches — which ordinary incident logging discards because "nothing happened."
  • Reconstruct where noticing thinned. Walk the detection chain of each case to find the specific weak link: the cue that didn't stand out, the threshold set wrong, the scan that didn't cover it, the fatigue that dulled the watcher.
  • Feed the fix back. Route each finding to the mechanism that owns it — cue design, scan field, threshold, readiness — so the near-miss changes the system, not just the file.
  • Tune misses and false alarms together. Track both, because tightening to catch more real signals also raises false alarms; the loop manages the trade rather than optimizing one blind to the other.

Tuning parameters

When it helps, and when it misleads

Its strength is turning luck into design. A near-miss is a failure that hasn't cost anything yet — the cheapest possible lesson — and a system that harvests them improves its noticing before the miss that isn't caught.
