Retrospective review — instantiates Cohort-Structured Replenishment Stabilization
After a cohort comes in weak, reconstructs the early conditions that caused it and feeds the lesson into a standing review of replenishment policy, so the same window failure is not repeated cohort after cohort.
A weak cohort is a fact; a repeated weak cohort is a policy failure. Recruitment-Failure Postmortem exists to convert the first into a defense against the second. A hospital system has a nursing-hire year come in badly short and underqualified. The immediate contingency covers the gap, but the Recruitment-Failure Postmortem asks the different question: why was this cohort weak? Reconstructing the recruitment window, it traces the failure to a clinical-placement bottleneck that throttled qualified applicants during the intake period — a systemic cause, not bad luck. The finding becomes a policy change (expand placement capacity ahead of the hiring window) and, crucially, an annual review of the replenishment pipeline so the same bottleneck is caught before it repeats. The reconstruction follows a disciplined after-action review[1]: what was supposed to happen, what did, why the gap, what changes.
The output is a cause-anchored policy change with a review cadence attached — the mechanism that stops the archetype's system from re-suffering the same recruitment failure every few years.
How it works¶
- Trigger on a confirmed weak cohort. The postmortem launches when strength data confirm a failure, not on a hunch.
- Reconstruct the early exposure causally. It rebuilds the conditions the cohort formed under and works back to root cause, distinguishing systemic causes from one-off bad luck.
- Convert findings into policy. Each cause is tied to a concrete change with an owner, not a report that gathers dust.
- Set and hold the review cadence. It establishes the recurring interval at which replenishment policy is re-examined, so learning is periodic rather than one-off.
Tuning parameters¶
- Launch threshold — how weak a cohort must be to trigger a postmortem; low thresholds learn more and cost more attention.
- Root-cause depth — proximate cause vs. systemic cause; stopping shallow gives quick fixes that miss the real driver.
- Blameless vs. accountability framing — blameless surfaces honest causes; accountability may deter candor but assigns ownership.
- Review cadence length — how often replenishment policy is re-reviewed; too frequent is noise, too rare lets drift accumulate.
- Findings-to-policy binding — how strongly a finding must produce a change; weak binding turns reviews into theater.
When it helps, and when it misleads¶
Its strength is durability: it is the only mechanism in the set whose job is to make the system learn, converting an expensive one-time failure into a standing policy safeguard and a review that does not quietly lapse.
It misleads through hindsight and narrative bias — a tidy, satisfying story of "what went wrong" that is not actually the cause, which then hardens into policy — and through ritual reviews with no teeth that produce documents but no change. The classic misuse is running the postmortem to assign blame rather than to fix the window, which poisons candor and buries the real cause. The discipline is to keep it blameless and evidence-based, tie every finding to an owned policy change, and audit whether past findings actually prevented recurrence.
