Spillover After-Action Review is the structured post-event review that reconstructs the real spillover path — which non-intended audiences picked the signal up, how it travelled, what reactions it drew, and where the plan was wrong — and converts it into a durable learning record plus corrections to the permeability estimates. Its defining role is closing the loop: it is the only mechanism in the set that makes the whole apparatus smarter over time, turning one event's surprises into calibration for every future map, scorecard, and monitor. It looks backward to feed forward; it does no live detection or response.

Example

A corporation's HR team sends an internal memo tightening its return-to-office policy, intended for employees only. It leaks: a screenshot reaches a professional social network within a day, a business-news reporter picks it up, and it becomes a public story about the company's culture — reaching prospective recruits, investors, and competitors the memo was never meant for.

Weeks later, the after-action review reconstructs what actually happened. The leak channel was a screenshot to a public network — a boundary the pre-release scorecard had rated low permeability, and got wrong. The amplifier was a single reporter. The real reactions ran ahead of the response, which had been sized for an internal audience. The review captures the durable lesson — "internal all-hands memos are effectively public documents; score that boundary high" — and pushes the corrected permeability estimate back upstream, so the next sensitive internal signal is planned as if the whole world may read it, because on the evidence it may.

How it works

  • Reconstruct the actual timeline. Trace the real path the signal took — first leak, channel, amplifier, reactions — rather than the path the plan assumed.
  • Compare predicted vs actual, boundary by boundary. Set the pre-release permeability scores against what really happened and mark every boundary that was mis-scored, especially the "low" ones that leaked.
  • Capture the surprises as reusable lessons. Record the channels, audiences, and reactions no one foresaw in a form the next planner will actually read.
  • Push at least one correction upstream. A review that does not update a scorecard, a map, or a sentinel's coverage has not closed the loop.

Tuning parameters

  • Review scope — this single event or a pattern across several. Single-event reviews are sharp and timely; cross-event reviews catch the systematic bias one event hides.
  • Depth vs cadence — deep occasional reviews or light frequent ones. Depth surfaces root causes; cadence keeps lessons fresh and the habit alive.
  • Blame vs learning framing — how much the review assigns fault versus extracts lessons. A learning frame keeps people reporting near-misses; a blame frame drives them underground.
  • Feedback destination — which upstream mechanisms receive the corrections. Routing lessons to the scorecard, the map, or the sentinel decides whether learning changes anything.

When it helps, and when it misleads

Its strength is that it is the system's only source of learning: it catches the boundaries everyone mis-scored and converts an expensive surprise into a cheaper next time. Without it, the same low-scored boundary leaks event after event.

Its failure mode is hindsight bias[n1] — in retrospect the spillover looks obvious and inevitable, which distorts the lesson into "we should have known" instead of "here is what we could not see" — and a review that assigns blame stops people reporting the near-misses that hold the richest lessons. Its classic misuse is a ritual write-up that no one reads and nothing feeds back into. The discipline that guards against this is to separate learning from accountability and to require every review to push at least one concrete correction upstream, so the apparatus actually recalibrates rather than merely reminiscing.
