The Diagnostic Story

Symptom: Contrary evidence accumulates but behavior does not change. People acknowledge a finding, then quietly narrow the original claim rather than updating their confidence. Reviews become defenses of prior decisions rather than genuine examinations of new data. The same surprise repeats because there is no institutional trace of the last time this belief was challenged.

Pivot: Install an explicit revision path: make the current belief and its triggers visible, define what kind of evidence would change confidence or scope, lower the social cost of changing your mind, and require that a changed belief have changed implications for action.

Resolution: Legitimate updating happens faster and with less rationalization. People can revise without humiliation, and the organization accumulates a usable record of what was believed, what changed it, and what decisions followed.
