The Diagnostic Story¶
Symptom: Participants report different experiences not because the conditions differ but because they know which condition they received. Staff treat groups unequally even when the protocol requires identical handling. Reviewers and graders show systematic differences once source or assignment becomes visible. Outcome assessments carry comments that reveal the assessor already knew which group was expected to do better.
Pivot: Install role-specific masking around the comparison: identify the expectancy pathways, decide which roles can be blinded, hide or neutralize condition identity and expectation-generating cues for those roles, keep comparator experiences credible, and monitor blind integrity — while keeping each role informed enough to act safely and ethically.
Resolution: Placebo, nocebo, enthusiasm, allegiance, halo, and observer-expectancy effects diminish.
