Target a small, strategic intervention point where change produces disproportionate system-wide effects.
The Diagnostic Story¶
Symptom: More effort, staffing, and monitoring have been applied, but the target pattern keeps returning. The same downstream problem reappears in many locations, suggesting something upstream is producing it. Small informal workarounds by a handful of actors already produce outsized effects, revealing where the real leverage is — but no one has named it or acted on it deliberately.
Pivot: Map the system well enough to identify the point where a small, bounded change should produce disproportionate system-wide effects — a rule, default, bottleneck, incentive, feedback, information flow, or goal that shapes many downstream actions. Apply the change there, and build in feedback monitoring from the start.
Resolution: Impact per unit of effort rises because action is concentrated where causes propagate rather than where symptoms appear. The causal pathway from point to system effect is explicit and revisable, and side effects are treated as core feedback rather than afterthoughts.

What this problem means
The structural problem is a mismatch between where people apply effort and where the system actually reproduces its behavior. Symptom-level action may reduce pain locally, but the pattern returns because the maintaining structure remains untouched.
