Accountability Chain Review¶
Traceability review — instantiates Scapegoat Displacement Interruption
Traces a failure back along its chain of ownership and decision authority, then routes responsibility to a legitimate forum and to repair.
Accountability Chain Review is the routing step: given a picture of what went wrong, it follows the chain of ownership — who owned which decision, which control, which handoff — and delivers responsibility to a legitimate forum and an owned repair. Where a causal workshop builds the picture of contributions, this review takes a settled picture and asks the accountability question of it: for each link, who had the authority and the duty, and where should this go to be adjudicated fairly and put right? Its defining property is that it produces destinations, not a map and not a verdict — a legitimate path for the responsibility question to travel, and a named owner for each piece of repair — so that interrupting a scapegoat never leaves a group with no route to accountability at all.
Example¶
A packaged-foods company issues a recall after a contaminated ingredient reaches store shelves, and public pressure wants a name — the line supervisor who ran the shift. An accountability chain review traces the actual ownership chain instead. It walks the failure link by link: the supplier who shipped an off-spec lot; the incoming-inspection step that was owned by quality assurance, not the line; the procurement decision to single-source that ingredient to cut cost; the escalation path that existed on paper but had no owner after a reorg. For each link the review records who held the authority and the duty, and where the question belongs — the supplier relationship to a contract-remedy forum, the single-sourcing decision to the risk committee, the broken escalation path to operations leadership. It then assigns repair: the recall logistics, the supplier corrective action, the restored escalation owner, and support for affected customers. The line supervisor, it turns out, owned none of the failed controls. The review's output is not "who to punish" but "where each piece of this legitimately goes, and who now owns fixing it."
How it works¶
- Walk the chain link by link. Follow the failure backward through owners, decisions, controls, and handoffs, asking at each link who held the authority and the duty.
- Match each link to a legitimate forum. Route each responsibility question to the venue that can fairly hear it — incident review, contract remedy, risk committee, adjudication, restorative process — with evidence standards, voice, and appeal.
- Assign repair ownership. For each piece of harm, name who now owns the remedy, the reintegration, and the follow-through, so repair does not become everyone's job and therefore no one's.
- Close the loop. Confirm every link has both a forum and a repair owner, so nothing falls into the gap between "not this scapegoat" and "no one."
Tuning parameters¶
- Chain resolution — how finely the ownership chain is decomposed. Finer resolution finds the real owner of a buried control but can fragment accountability into unactionable pieces.
- Forum formality — from a lightweight internal review to a formal adjudication. More formality confers legitimacy and due process but costs speed and can feel adversarial.
- Repair-ownership strictness — whether every remedy must have a single named owner. Strict single-ownership drives follow-through but can miss genuinely shared duties.
- Escalation reach — how far up the chain the review is permitted to route. Limiting reach protects senior actors from scrutiny; extending it risks over-reaching on thin links.
When it helps, and when it misleads¶
Its strength is that it prevents the quiet failure of anti-scapegoating work — leaving people with a dissolved villain but no legitimate route to accountability or repair. By separating the frontline operator from the owner of the failed control, it resists the reflex to blame the visible person. Its natural anchor is the sharp end and blunt end,[n1] the distinction between the operator at the point of action and the organisational context that set them up — the review's whole job is to trace from the sharp end back to the blunt.
Its failure mode is over-reach and under-reach in equal measure: a chain review can be gamed to stop conveniently one link below the real decision-maker, letting the blunt end escape; or it can fragment ownership so finely that accountability evaporates into a diagram. It can also route a case to a forum designed to bury it. The discipline is that the chain must be followed to where authority actually sat, not to where it is politically comfortable to stop, and that every routed question lands in a forum with real standards, voice, and appeal — routing is only legitimate if the destination is.
