Governance review — instantiates Attrition and Dropout Monitoring
An independent body that periodically reads the attrition evidence against pre-set triggers and decides whether to continue, adapt, or stop when loss threatens the inference or the participants.
Detection and action mean little if no one with authority is willing to call a halt. Data Monitoring Review supplies that authority. It is a periodic, independent adjudication in which a body not running the study reads the accumulated attrition and safety evidence against pre-specified triggers and issues a binding recommendation: continue, adapt, or stop. Its defining move is independence with teeth — the reviewers have no stake in the study's success and hold a decision the operating team structurally cannot make about itself, which is what lets a pre-committed threshold survive contact with a team that badly wants to keep going. It converts monitoring into governed action. It is not an instrument that detects loss and not a field routine that recovers it; it is the gate that decides what the detected loss means for the study's continuation.
Example¶
A multi-center cardiovascular outcomes trial charters a data monitoring committee, independent of the sponsor and investigators, to meet at scheduled interim looks. At its third look the committee reads the dashboards and analyses prepared for it and sees loss-to-follow-up climbing past its pre-set threshold and concentrating in one region's sites. It does not run the analysis itself and it does not phone participants; it decides. Invoking the response policy written into its charter, it mandates enhanced retention at the affected sites, requests a formal sensitivity analysis before the next look, and reaffirms the stopping boundary that will end the trial if follow-up keeps degrading. Because those triggers were fixed in advance and the committee is independent, the decision reads as governance rather than as a team rationalizing its way past a problem.
How it works¶
Its distinguishing discipline is pre-committed, independent adjudication:
- Charter independence and authority. Members sit outside the study's success; their recommendations bind.
- Meet on a schedule of interim looks, so decisions arrive in time to matter rather than only at the end.
- Read others' outputs, do not generate them. The committee consumes the dashboard, balance table, and sensitivity analysis prepared for it and judges the whole picture.
- Match evidence to a pre-specified trigger-and-stopping policy. Crossings invoke defined responses — enhanced retention, mandated analysis, protocol change, or a stopping boundary — decided against thresholds set before the data were seen.
Tuning parameters¶
- Review cadence — how frequent the interim looks are; more catch fast collapse but add overhead and repeated-looks statistical cost.
- Trigger thresholds — how much attrition invokes which response; tighter thresholds act sooner but escalate on more false alarms.
- Stopping-boundary strictness — how conservative the rule for halting is, balancing early protection against stopping on noise.
- Membership independence — how strictly conflicts are excluded; stronger independence resists rationalization but is harder to staff.
- Scope — safety-only versus safety-plus-validity; a wider remit catches inferential collapse but demands more of the committee.
When it helps, and when it misleads¶
Its strength is that pre-committed, independent decisions resist post-hoc rationalization: a stopping rule fixed in advance and enforced by disinterested reviewers is far harder to argue around than a judgment the invested team makes about its own study, and it protects participants when attrition signals harm.
It misleads when thresholds are so rigid they cannot accommodate a novel ethical concern the charter never anticipated, or when reviews are so infrequent that a fast collapse runs its course between looks. The classic misuse is a committee that is independent in name only — populated by insiders who rubber-stamp continuation — which imitates the form of a data and safety monitoring board[n1] while discarding the independence that gives it force. The guarding discipline is genuine charter independence, thresholds and stopping boundaries pre-specified but with explicit room for an ethical override, and a cadence matched to how fast the study can go wrong.
