Asks the people who left why they left — in their own words, coded but uncertainty-preserving — so a withdrawal is recorded as a diagnosis rather than a blank.
A dropout row that reads only "lost" tells you nothing about whether the loss threatens the study. Withdrawal Reason Survey or Interview is the instrument that fills that blank with evidence. It asks a departing or departed unit why — through a short questionnaire or a brief conversation — and captures the answer in the unit's own words before coding it into a reason taxonomy. Its defining move is that it treats a withdrawal as a source to be interviewed, not a number to be counted, and it is scrupulous about whose account a reason is: a reason volunteered by the participant, a reason inferred by staff, and an administrative label like "inactive account" are recorded as three different kinds of thing. That single discipline — preserving the uncertainty in a reason rather than flattening it — is what separates diagnosable attrition from a guess dressed up as a code.
Example¶
A company piloting a productivity-tracking system watches morale-survey responses dry up among the employees assigned to it. The raw log would call these "non-responders" and move on. Instead a short, voluntary exit interview goes out. Some volunteer that they "felt surveilled" and stopped answering on principle; a few had simply left the company (an administrative reason the HR system confirms); staff infer, from timing, that a cluster stopped after a critical all-hands, but no participant said so. Coded faithfully, the picture is not "low response rate" but "a chunk of the missingness is a reaction to the intervention itself" — flagged as participant-reported — sitting beside a separate, clearly-labelled administrative slice and a tentative, staff-inferred one. The blank has become a diagnosis, with its confidence attached.
How it works¶
Its distinguishing discipline is capturing reasons with their provenance:
- Trigger on withdrawal, stay light and voluntary. The instrument is short, low-burden, and never conditions anything on answering — asking why someone left must not itself become a reason to leave.
- Open-ended first, then coded. Participants answer in their own words; those words are mapped to a reason taxonomy afterward, so the code follows the account rather than leading it.
- Tag the source of every reason. Self-reported, staff-inferred, and administrative reasons are marked as such and never merged.
- Write the reason and contact status back to the loss record. Each event's reason and reachability fields are populated, so downstream mechanisms inherit a diagnosed, not blank, withdrawal.
Tuning parameters¶
- Reason-code resolution — how fine the taxonomy is; more categories aid diagnosis but invite false precision and privacy exposure.
- Mode — anonymous survey versus interview; interviews surface richer, harder-to-code reasons, surveys scale and reduce social pressure.
- Timing — asking at the moment of withdrawal versus later; sooner captures fresher reasons, later may reach people who have cooled off.
- Coder and provenance rules — who assigns codes and how aggressively staff may infer a reason no one stated.
- Anonymity level — how much identifying context is retained; more candor against less linkage to the rest of the record.
When it helps, and when it misleads¶
Its strength is diagnostic: it is the only mechanism that can tell whether loss is administrative, practical, burden-related, harm-related, or outcome-related — the last of which is the signature of the dangerous, non-ignorable missingness that biases a comparison. A withdrawal that turns out to cluster around side effects or dissatisfaction is a validity warning a bare count would have hidden.
It misleads when reasons are inferred from thin evidence and recorded as if known — misclassified attrition — or when social-desirability bias[1] leads people to give the polite, acceptable reason ("scheduling") over the real one ("the tracking felt invasive"). The guarding discipline is to preserve uncertainty everywhere: mark inferred reasons as inferred, keep an explicit "unknown" code rather than forcing a guess, and treat a stated reason as evidence about the missingness mechanism, not proof of it.

It does not statistically compare the people who left against those who stayed on baseline covariates (completer_non_completer_contrast, its nearest twin Completer Balance Table) — the survey asks leavers directly rather than measuring them — nor decide the formal missingness interpretation those reasons point to (missingness_assumption_record, Missing-Data Sensitivity Analysis).

The table measures leavers from data the study already holds (they were younger, lower-scoring, higher-burden); the survey asks leavers for an account the data cannot contain (they felt surveilled). One is external and covariate-based, the other is self-reported and motive-based — and a serious study wants both, because a reason and a measured difference check each other.
