No-Fault Learning Review¶
Facilitation ritual — instantiates Regret-Signal Calibration
A blameless review that rebuilds what was known and reasoned at the decision point and separates controllable choices from bad luck, so people surface information instead of hiding it.
A review that assigns blame teaches everyone in the room to withhold. The No-Fault Learning Review is built around a single conviction — that the honest account of a regretted decision is more valuable than a culprit — and it protects that account by design. Before anyone evaluates the choice, the ritual reconstructs the decision-time frame (what was known, what was uncertain, what options were live) and then draws a clean line between what the decision-maker actually controlled and what was luck, missing information, or system design. Its output is not a verdict and not a rule change; it is a fair, shared, decision-time-faithful record that a downstream mechanism can safely learn from. The whole point is that people volunteer the awkward truth precisely because volunteering it will not be punished.
Example¶
A regional airline runs a review after a flight executed a go-around from an unstabilized approach — a safe outcome, but a scare. A blame-first debrief would land on "the captain descended past the gate." The no-fault ritual instead rebuilds the moment: the weather data the crew actually had, a late runway change from ATC, and the workload spike that came with it. It then sorts responsibility — the crew controllably continued past the stabilization gate, but the late runway change and the ambiguous company callout were system-and-luck factors. Because no one is being hung for it, the first officer volunteers that the airline's stabilization-callout wording is genuinely ambiguous under pressure — exactly the kind of information a punitive review would have buried. That single disclosure is worth more than a reprimand.
How it works¶
The ritual enforces strict sequencing. Reconstruction before evaluation: the decision-time information set is written down first, so hindsight cannot leak into the account. Foresight, not hindsight: the choice is judged only against what was knowable then. A controllability ledger: each factor is filed as controllable choice, bad luck, missing information, or system design. A no-name-blame norm: the facilitator forbids person-directed fault language and redirects it to process and information. What leaves the room is a reconstructed rationale and a responsibility map — the raw material for learning, not the learning itself.
Tuning parameters¶
- Psychological-safety strictness — how firmly person-blame is ruled out. Tighter norms surface more information but can feel like accountability is being dodged if not paired with a clear recklessness line.
- Reconstruction depth — how exhaustively the decision-time frame is rebuilt. Deeper is fairer and slower.
- Room composition — who attends. Including the decision-makers gets tacit context; including their managers can chill disclosure.
- Attribution granularity — how finely controllable-vs-luck is split. Finer is fairer but invites endless litigation of edge cases.
When it helps, and when it misleads¶
It is the right instrument in exactly the environments the archetype warns about: places punitive enough that regret talk will otherwise suppress information rather than improve it. By trading blame for disclosure, it keeps the signal flowing.
Its failure mode is that "no-fault" slides into "no-accountability." Blamelessness is meant to protect honest error and system-induced mistakes — not to launder negligence or repeated recklessness. A classic misuse is a team that shields a colleague's genuinely reckless behavior under the blameless banner, review after review. The guarding discipline comes from just culture,[n1] which draws a firm line: console honest error, coach at-risk shortcuts, but hold reckless conduct accountable. Blamelessness ends where recklessness begins.
