The Regret Pre-Mortem is the only mechanism in this family that runs before the die is cast. It manufactures regret on purpose, in advance, to use as a design input. The move is deliberately backward: instead of asking "what might go wrong," it asserts that the decision has already gone wrong — "it is a year later and we deeply regret this" — and asks the group to explain why. Assuming the failure as a settled fact loosens more, and more concrete, failure modes than forward forecasting ever does. The anticipated regrets are then mapped to the stakeholders who would suffer them, ranked, and — only for the ones worth it — hedged with a cheap option-preserving move made now, while options are still cheap.

Example

A city government is about to launch a new online permitting system as a single big-bang cutover. The pre-mortem convenes the team and states the failure flatly: it is a year from now and the launch is a fiasco we regret — what happened? The room fills quickly: contractors couldn't submit at all, there was no way to roll back, and the smallest firms were hit hardest because they had no slack to absorb downtime. Each regret is mapped to who bears it — contractors, permit clerks, small businesses, the IT team. The highest-plausibility, highest-pain regret is the absent rollback. The response is a low-cost option-preserving hedge decided before commitment: keep the legacy system running in parallel for sixty days (illustrative). The regret is designed out while doing so is still nearly free.

How it works

The exercise runs in three beats. First, prospective hindsight[n1] — assume the plan has failed and narrate the causes, which is psychologically easier and more productive than predicting them forward. Second, rank the imagined regrets by plausibility crossed with stakeholder pain, using the stakeholder map to see whose regret each one is. Third — and only for the top-ranked, hard-to-reverse regrets — design a cheap hedge that preserves an option: a rollback path, a staged rollout, a reversible pilot. The exercise stops at option design; it does not operate any later decision.

Tuning parameters

  • Imagination horizon — how far into the future the failure is set. Distant horizons surface strategic regrets; near ones surface operational ones.
  • Scenario count — how many distinct failure narratives to generate before ranking. More breadth, more time.
  • Plausibility bar — how likely an imagined regret must be to earn a hedge. Set it low and you hedge against ghosts.
  • Hedge budget — how much option-preserving cost is justified up front. Over-hedging is its own expense.

When it helps, and when it misleads

Its strength shows most when a decision is costly or hard to reverse — exactly the archetype's trigger for anticipated regret. Pulling the regret forward turns it into cheap design guidance instead of expensive hindsight.

Its failure mode is letting vivid regrets outrank likely ones: a lurid, improbable disaster hijacks the ranking and the team freezes a bold-but-reversible move it should have made — the archetype's warning that "leaders avoid bold but reversible experiments because they overweight vivid anticipated regret." Over-hedging is the paired misuse — guarding every imagined regret until the hedges cost more than the risks. The guarding discipline is to weight by plausibility rather than vividness, and to reserve hedges for regrets that are both irreversible and high-pain.
