The Diagnostic Story

Symptom: The numbers look acceptable in the aggregate but a subgroup is failing badly, or an individual case looks alarming but the system-level picture is fine — and the team keeps arguing past each other because each side is reasoning from a different level. Averages hide the variation that matters; anecdotes can't be compared to population data. Interventions chosen at one level misfire because the real causal structure lives at another.

Pivot: Name the scale currently in use, then test what adjacent or alternative scales reveal. Compare what each level shows and what it compresses away. Choose the level that should govern the decision, then translate the insight into action at the level where action is actually possible.

Resolution: The causal attribution becomes more accurate because the analysis is done at the level where the decisive variation lives. Interventions fit better because they target the right level. The local-global confusion that generates persistent misdiagnosis is replaced by a deliberate, translatable choice of analytical frame.

Scale mismatch is subtle because the current view often contains real evidence. The problem is not necessarily false information; it is evidence made misleading by the level at which it is organized.
