Review workflow — instantiates Cognitive Workflow Sequencing
Splits a group review into ordered phases — orient, review evidence, interpret, challenge, synthesize, decide — so complex judgment cannot collapse into immediate advocacy.
A Structured Review Sequence runs a group review as separated, ordered phases with a chair who enforces the boundaries between them, so that no one interprets before the evidence is on the table and no one advocates before the interpretation has been offered independently. Its single defining move is keeping the phases from bleeding into each other: fact-gathering closes before hypothesizing opens, and a hypothesis is challenged before it is allowed to become a finding. Where a generic meeting lets the senior or loudest voice frame the problem in the first minute, this workflow puts orientation, evidence, interpretation, challenge, synthesis, and decision in a dependency order and holds the room to it.
Example¶
A regional airline's flight-safety board convenes after a runway-incursion near-miss. Left unstructured, the meeting would open with the chief pilot declaring "obviously the first officer misread the taxi diagram" — an interpretation that would anchor the whole room before a single fact was checked. Instead the chair runs it in phases. Orientation: a bare timeline of what happened, facts only, no theories. Evidence review: the ATC transcript, the flight-data recorder trace, ground-radar, and weather are laid out and reconciled — still no hypotheses allowed. Interpretation: each board member writes their reading of the cause privately before anyone speaks. Challenge: a designated member is tasked with arguing against the leading hypothesis. During that challenge, a contradictory radio timestamp surfaces, and the chair calls the group back into the evidence phase to reconcile it — a backtrack rather than a suppressed inconvenience. Only then: synthesis and decision (findings plus corrective actions). The outcome is a set of findings resting on reconstructed facts, not on the chief pilot's first instinct.
How it works¶
- Named phase boundaries. Each phase has an explicit entry and exit; the group is only ever doing one kind of cognitive work at a time.
- Evidence closes before interpretation opens. Hypotheses are barred during fact-gathering, which is what prevents early theories from steering which facts get attention.
- Challenge is mandatory, not optional. A dissent role is assigned so the leading hypothesis is stress-tested before it hardens into a finding.
- Backtracking is explicit. New evidence appearing during challenge or synthesis reopens the evidence phase; the chair calls the return rather than forcing forward motion.
- The chair polices boundaries. The sequence owner enforces "not yet," protects independent interpretation, and decides when a backtrack is warranted.
Tuning parameters¶
- Phase granularity — a few broad phases versus many fine ones; add phases for high-stakes or contentious reviews, collapse them for routine ones.
- Evidence-gate hardness — whether interpretation is hard-blocked until evidence is declared complete, or merely discouraged; harder gates cost time but stop premature theorizing.
- Challenge formality — a rotating assigned devil's advocate versus open critique; formal dissent surfaces more but can feel adversarial.
- First-interpretation independence — written privately before discussion versus an open round-robin; privacy blunts anchoring at the cost of speed.
- Backtracking permissiveness — how readily new evidence reopens a closed phase versus pressing forward; discovery-heavy reviews need it loose, routine ones tight.
- Chair authority — soft prompts versus hard stops on phase violations.
When it helps, and when it misleads¶
Its strength is that it denies first-mover advocacy the power to set the frame: it separates fact from theory, and it makes a contradictory late fact reopen the record instead of getting buried. It is at its best in incident reviews, expert panels, and design reviews where the cost of a foregone conclusion is high.
Its failure mode is that the phases become theater. If the chair lets a senior member interpret during "evidence," or if the challenge role is a formality, or if the sequence hardens into a ritual that never actually backtracks, the structure launders a conclusion reached in advance rather than protecting the reasoning. This is the setting in which groupthink[n1] thrives — the appearance of orderly process substituting for genuine independent judgment. The guarding discipline is that the chair must protect independent interpretation and actually call backtracks when late evidence lands; a review that cannot reopen its own record has stopped being a review.
